Scheduling an Appointment

Patients, please note: Your insurance companies require us to make a copy of your insurance card and obtain updated paperwork every year. We understand that this may be an inconvenience, so in order to make this easier for you, you may print off the Patient Demographic Information sheets by clicking the link below and fill them out before your visit.
If you do not wish to provide updated paperwork or a copy of your insurance cards, we will be unable to file your claims to insurance and you will be responsible for the visit charge at the time of service.

Refilling a Prescription
If you need a prescription refilled you may Do so by calling our office (317) 329-7050
Please be prepared to provide the following information:
Patient's name and Date oF birth
a number where the patient can be reached
Name of Pharmacy and their telephone number
The name and strength or dose of the medication
Instructions for post-treatment care of Biopsies, freezings and Blue Light procedures can be found under the "tips for healthy Skin" Tab at the top of this page.
Medical Records Release
If you need your medical records released to a different medical office please print out and complete the release form below.
